Chemical Peels 101, Chapter 3

Having been in the practice of dermatology for over 30 years, I realized that there was a void in the industry for an effective chemical peel designed to lighten, brighten and firm the skin with minimal side effects and downtime. The cosmeceutical industry had several peels on the market that addressed sun damage and produced improvement, but they were often associated with pain and side effects. The mild alpha hydroxy acid and beta hydroxyl acid peels available were less painful, with no downtime; but their results were not very dramatic. Chemical peels with strong, caustic acids, like high concentrations of trichloracetic acid and phenol, caused significant tissue destruction, downtime, risks and variable outcomes. The challenge to me was to create an entirely new type of peel that treated discoloration and sun damage with significant results, minimal downtime, and few side effects. I created the Poly-Organic Brightening Peel.

Poly-Organic Brightening Peel

Unlike the traditional exfoliating peels, Rx Systems PF’s Poly-Organic Brightening Peel uses a unique blend of safe, but effective, organic acids to cause natural chemical exfoliation of the stratum corneum and epidermis. The peel contains antioxidants and skin lightening and brightening ingredients, in addition to the combination of exfoliative and reparative organic acids (organic compounds are naturally occurring molecules containing carbon but may have other elements like hydrogen, oxygen and nitrogen). The synergist effect of these ingredients creates a comprehensive reparative and rejuvenating chemical peel, which can be combined with other cosmeceutical products and treatments. This special formulation includes a unique, innovative blend of peeling agents:

alpha-hydroxy acid (glycolic acid)
beta-hydroxy acid (salicylic acid)
dicarboxylic acid (azelaic acid)
an aromatic organic molecule (resorcinol)
hydroxyl-pyrone (kojic acid)

It can be combined with L-Ascorbic Acid or Retinoic Acid (physicians only) to individualize the benefits for certain patients.

While results are predictable and noticeable after just one 20-minute Poly-Organic Brightening Peel, more dramatic, optimum results are seen after a series of peels. The number of treatments needed by a patient depends upon the problem being addressed, but usually ranges from three to six treatments spaced two to four weeks apart. Other products and treatments can be used in conjunction with the peel, depending on the skin problems being treated.

Maintaining Peel Results

The changes that occur in the epidermis following a chemical peel last for years. In order to maintain the results, it is critical to avoid sun damage. As with any treatment, it is always important to protect the skin from harmful UV rays after the peel, as this can aggravate and cause damage to the skin. It is recommended that peel recipients use an SPF 30 or higher.

The patient’s skin type, skin care before and after the peel, the doctor's level of experience, and the patient’s lifestyle after the procedure can also affect the results. Some types of skin problems respond better to chemical peeling than others. People with lighter skin who limit their sun exposure after the procedure tend to have better results than those with darker skin and those who continue to spend lots of time in the sun.

Options for skin resurfacing

Chemical peels, dermabrasion, and laser resurfacing are the most commonly used techniques for improving the texture and appearance of the skin. Although these techniques use different methods, they have basically the same effect on the skin –they destroy and remove the upper layers of skin to allow for skin regrowth.

No one technique is necessarily better than the others. When done by an experienced surgeon, laser resurfacing may be slightly more precise than chemical peeling or dermabrasion. But the choice of technique is based on the site the patient wants to treat, the patient’s skin type and condition, the doctor's experience, the patient’s preferences and other factors. Some people may get the best results using a combination of techniques.

Chemical skin peels can improve skin texture and reduce the appearance of fine lines and wrinkles. They work by removing the outermost layers of the skin damaged by sun exposure, age, and skin conditions to allow smooth, healthy new skin underneath to grow. Skin peels may be superficial (mild), medium or deep.

The ideal candidate for chemical peels is someone who:

Is in good physical health
Is psychologically stable
Understands the procedure
Does not smoke
Wants to look younger and more refreshed
Is unhappy with the appearance of wrinkles or brown spots, especially around the mouth or eyes
Is free of active skin infections, including most kinds of acne
Is informed about the appropriate skin tone for the type of peel used
Is not taking Accutane or has not taken it for the previous 18 months.
Is free from large or unusual scar formations, such as keloids
Is realistic in his/her expectations
Is willing to accept the limitations involved in the healing process

Peel Side Effects and Risks -

Significant complications with chemical peels are infrequent, but potential complications include:

Scarring
Infection
Temporary or permanent changes in skin tone or uneven tone
Hyperpigmentation (a darkening of the skin, treatable in most cases with current bleaching techniques)
Hypopigmentation (a lightening of the skin, more difficult to treat)
Cold sore breakouts in patients who have a history of recurring blisters and cold sores, like herpes and shingles (an anti-viral medication before the procedure can help prevent this)
Risks for patients with a family history of heart disease

A chemical peel (except for a superficial peel) may not be done if a patient has:

Recently used isotretinoin (a drug used to treat acne)
Had recent facial surgery or facial radiation therapy. This can make regrowth of the skin more difficult
An active herpes infection affecting the area to be treated
An impaired immune system. This can delay healing and increase the risk of infection and skin color changes after the peel
Known allergies to certain medicines

Patients can help minimize certain risks by following the advice and instructions of their doctor, both before and after the chemical peel. Pre-existing conditions may also put a patient at risk. Patients who have a history of heart disease or herpes, have a tendency to form large or unusual scars, or who have undergone considerable amounts of radiation or facial x-rays should consult their doctor before having a chemical peel.

Managing Expectations -

Some types of skin problems respond better to chemical peeling than others. People with lighter skin who limit their sun exposure after the procedure tend to have better results than those with darker skin and those who continue to spend lots of time in the sun. Changes in the color and texture of the skin caused by aging and sun exposure may continue to develop after a chemical peel. Chemical peels are not a permanent solution for these problems.

Chemical peels are designed to wound and remove the upper layers of the skin. Patients will need to prepare themselves for how their skin will look immediately after the peel and throughout the healing process. Patients will also need to be prepared to use cosmetics to blend skin tones between treated and untreated areas, such as between the face and jaw line.

During the early healing period after a chemical peel (before the skin has finished peeling), patients will need to avoid sun exposure. After the early healing period has passed, patients will need to wear sunscreen every day and limit sun exposure as much as possible. New skin is more susceptible to damage and discoloration from sunlight.

Even with realistic expectations, patients may not see results for several weeks or months after a chemical peel.

Non-Surgical Facelift for Those Who Hate Needles and Knives

Aging and its effects on a person’s facial appearance can be a hard pill to swallow for some, but with a non-surgical facelift, a youthful appearance is possible through science…and without pain.

Cleanse

Back before we knew about the Poly-Organic Brightening Peel, I had ordered a VI Peel to try. I then ordered the Poly-Organic Brightening Peel and decided to go make a direct comparison between the two peels.

The Scoop on Sunscreen, Part 2

There is definitely a role for antioxidants in the area of sun protection. Ultraviolet rays create free radicals that cause photoaging of the skin. It has been demonstrated that the skin’s native antioxidant protection breaks down during excessive UV injury and aging (due to the fact that there are more free radicals created by the UV damage and cell metabolism than there are antioxidant molecules available in the skin). This allows free radicals to damage DNA, proteins, and lipids.

The photo-protective capability of antioxidants does not lie in their ability to reflect or absorb UV rays. Rather, they prevent the damage created by UV-generated free radicals. Antioxidants neutralize free radicals by donating one of their own electrons, ending the destructive cascade. The antioxidants themselves don’t become free radicals, because they are stable in either form due to their chemical behavior. They act as scavengers to prevent cell damage. Antioxidants that give up their electrons are not harmful, because they have the ability to accommodate the change in electrons without becoming reactive.

Under normal conditions, the antioxidant defense system within the body can easily handle free radicals that are produced. A free radical attack on a membrane usually damages a cell to the point that it must be removed by the immune system. Free radicals are commonly associated with an inflammatory response and are hypothesized to be greatest 24 hours after an insult. The immune system is the main body system that utilizes free radicals.

New methods to protect skin from photodamage are necessary, as sunscreens are under-used and have incomplete spectral protection. Skin naturally uses antioxidants to protect itself from photodamage. Vitamin C, Vitamin E, alpha lipoic acid, retinol, selenium, zinc, solymarin, soy, soflavones, and tea polyphenols may supplement sunscreen protection and provide additional photodamage repair when sunscreens fail. The incorporation of antioxidants into sunscreens provides added protection against the harmful effects of UV exposure and may provide reparative effects if/when sunscreens fail.

Antioxidants are manufactured in the body and can also be applied to the skin or extracted from ingested food. In other words, they create an increase in photoprotection when used in conjunc- tion with sunscreens.

There are other things that can be done in addition to wearing an effective sunscreen to protect against UVA/UVB rays. Particularly if you are reluctant to wear sunscreens, I would encourage you to wear protective clothing, find shade, avoid tanning beds, and get Vitamin D safely. Keep in mind that snow, ice, sand, glass and metal reflect UV rays. UV rays are able to penetrate water to a depth of about 30 inches. It is important to remember that fog and clouds only reduce UV intensity about 30 percent, and the sunscreen benefit of clothing is related to the thickness, type of fiber, weave, stretch, color and other factors. In fact, nylon stockings have an SPF of only 2-3.

Patients often inquire about window glass and sunglasses. UVB rays are blocked by glass, but UVA rays (the longer wavelength UV rays) pass through window glass and sunglasses and can damage the skin and eyes.

All the news is not bad. There have been advances. Environmental regulations have improved the thickness of the ozone layer. Clothing has been manufactured to provide sun protection, some chemically treated with sunscreen agents. Glass has been thickened, polarized and tinted to increase its sun protective properties. Many cosmetic products also contain pigment and sunscreen ingredients, which increase their sun protection ability to an SPF 3-15 based on the product.

Inorganic sunscreens—micronized zinc oxide and titanium dioxide—provide broad-spectrum protection with no concerns about toxicity. Emphasis on inorganic sunscreens and synergistic antioxidants may avoid concerns associated with organic sunscreens and may be particularly useful if you concerns about specific sunscreen ingredients.

The Scoop on Sunscreen, Part 1

Summer time is fast approaching, and although here in St. Louis glimpses of the sun recently have been far and few between, sunny days will return to us eventually. Although the use of sunscreens shouldn’t be reserved for just sunny, summer days, this is definitely the time of year when they are foremost in most people’s minds. Of course, if your concern includes the prevention of premature aging skin, you probably think about sunscreen year round, because UVB/UVA radiation is the primary preventable cause of aging skin.

To protect your skin from both types of radiation (UVB and UVA), a sunscreen must contain a combination of organic (chemical sunscreen ingredients) and inorganic (physical sunscreen ingredients) filters. Combination sunscreens enhance the SPF of the final product.

Chemical sunscreen ingredients act as an ultraviolet sponge to absorb damaging UV rays. The chemical reaction of the UV rays with the chemical sunscreen ingredient creates heat, which is why some people experience “feeling hot” when using sunscreens. Physical sunscreen ingredients deflect or block damaging UV rays in a similar fashion to protective clothing.

Current toxicology based on acute and chronic exposure to (UVB/UVA) broad-spectrum sunscreens concluded that sunscreen ingredients or products pose no human health concern, despite what may be reported in the media. What is a positive, as well, is that micronized, transparent zinc oxide is not absorbed into the skin; and there are no reports of skin allergies to zinc oxide.

Most dermatologists would agree that the use of sunscreen has contributed to a decrease in the incidence of photo-damage and pre-cancerous skin lesions. In fact, the American Academy of Dermatology recommends that, regardless of skin type, a broad-spectrum sunscreen with an SPF of at least 15 should be used year-round. Patients must understand that sunscreen shouldn’t be reserved for use only on sunny days. Seventy percent of the sun’s ultra-violet rays can pass through the clouds. Additionally, sand reflects 25 percent of the sun’s rays, and snow reflects 80 percent of the sun’s rays.

SPF is an indication of a sunscreen’s effectiveness in preventing sunburn related to the length of time in the sun, but it does not actually increase proportionately. An SPF of 35 blocks 97-98 percent of the UVB rays, while an SPF of 15 blocks 93 percent of the rays, and an SPF of 2 screens 50 percent of the UVB rays.

SPF only relates to a sunscreen’s ability to block UVB rays. Unfortunately, at the present time there is no measure to quantify the effectiveness of a sunscreen’s ability to block UVA rays. It is well known that chemical sunscreen ingredients that block UVA rays are somewhat unstable when exposed to UV rays and oxygen (air). This is further complicated by the fact that we do not have the ability to measure the stability or effectiveness of chemical sunscreens that block UVA rays.

The most reliable and effective ingredients for protecting the skin from UVA rays are physical sunscreen ingredients. Zinc oxide and titanium dioxide are the two physical blockers used here in the United States. Unlike years ago when physical blockers looked like a white paste on your face, nanotechnology has eliminated this problem by creating micronized particles of zinc oxide and titanium dioxide. Micronized zinc oxide and micronized titanium dioxide are transparent when applied to the skin.

The ideal sunscreen must be broad spectrum; that is, containing agents that can provide protection from both UVB and UVA radiation.

All FDA-approved sunscreen ingredients have a particular absorption spectrum. The ideal sunscreen would combine ingredients to expand the range of ultraviolet protection from damaging ultraviolet rays and should include either micronized zinc oxide or titanium dioxide, given the lack of knowledge regarding the effectiveness of chemical UVA blockers. In addition, it has been shown that daily use of a sunscreen increases its photo-protection, as sunscreen ingredients can bind to skin cells in the outer layer of the skin.

Research into Rosacea

In recognition of the fact that April is Rosacea Awareness Month, it seemed appropriate to talk a little about some of the recent research into the causes of rosacea, which will hopefully, in turn, lead to better treatments and prevention.

Honing in on one specific cause of rosacea may be impossible, however, researchers are now uncovering some promising theories and doing further studies to help solidify what they have found thus far.

The results of some of the research being done points to the following causes:

Blood vessel disorder
Genetic predisposition
Demodex folliculorum (skin mites)
pH balance
Cathelicidins

The theory behind the blood vessel disorder is that pimples and/or the tissue growth associated with phymatous rosacea is instigated by the fluid built up in blood vessels, causing them to expand. This fluid can set off inflammation. Structural abnormalities have been noted in the blood vessels of rosacea patients studied.

A lot of attention is being given to the study of Demodex folliculorum…skin mites…as another very viable cause of rosacea. It is a little disconcerting to think about mites living on our skin, but it is a reality that researchers believe makes rosacea a struggle for many. The diet of these mites consists of dead skin cells. The mites make their homes in our hair follicles on our bodies, preferably our faces. When studying the skin samples taken from those suffering from rosacea, researchers recently found the skin mite population of the study group to be much higher than those without rosacea.

As we recently discussed in a blog about acne, when sebaceous glands get clogged with oil and dead skin, inflammation (pimples) can occur. This reaction can also be caused by hair follicles clogged by skin mites. One study conducted by researchers in Ireland found that bacteria carried by the skin mites caused adverse reactions in rosacea patients and not in those who did not suffer with rosacea.

Another condition that can contribute to outbreaks of rosacea is the pH of the skin. A higher acidic pH aggravates rosacea. A couple of the things that can affect the acidity of the skin are the foods you eat, what you drink, and the amount of stress you are experiencing. Commonly known foods and drinks that can affect rosacea are those that are spicy (hot) and red wine. A more complete list is provided here.

Finally, on our list are cathelicidins, molecules that are a part of our bodies’ normal bacteria-fighting immune system. Our bodies routinely will call on cathelicidins to help in response to “dangers” that are sensed, such as spicy foods, stress, and high heat. A study by Dr. Richard Gallo from the University of California-San Diego helped to determine that the makeup of cathelicidins in rosacea patients is different and more abundant. According to the research, an additional enzyme, kallikrein, is present in large amounts, which stimulates the cathelicidin unnecessarily, causing skin inflammation. One therapy being looked at involves bringing cathelicidin levels into a normal range through the use of Vitamin D.

Although rosacea is a condition that can have life-long negative affects on patients’ lives, the research that is being done is promising in that, along with certain lifestyle changes, there may be new and better therapies on the horizon.

Poly-Organic Peel on the Way

A couple of weeks ago, I talked about “desquamation”, the process by which your body sheds old, dead skin cells naturally. This week I wanted to touch on chemical peels and how they can assist in removing dead skin while also addressing other skin issues.

As your skin ages and suffers years of sun damage, the texture of it can become rough, discolored, and it can begin to lose its firmness. Exfoliation can help, but normal exfoliation is a superficial process restricted to the stratum corneum. Chemical peels accelerate the normal desquamation process and generate a quicker exfoliation of the stratum corneum.

Chemical peels can be superficial, but based on the type of peel, they can also reach to a medium depth, affecting deeper cell layers in the epidermis. Medium-depth peels are used to treat discolored skin, hyperpigmentation, fine lines, wrinkles, sun damage and precancerous skin lesions. Medium-depth peels usually take between 30 and 60 minutes to administer. Side effects include moderate stinging or discomfort for 30-60 minutes.

The depth of peels depends on the active ingredient(s), the concentration of the active ingredient(s), the number of passes or layers applied, the type of skin (thin or thick), the type of skin preparation (eliminating residual sebum and creating an even pH), the application technique, the anatomic location, the application contact time and finally, the spacing of subsequent peels.

The way in which they work is that the chemical peels create a wound in the epidermis, which stimulates cell growth (the healing process) and the formation of important substances deficient in sun damaged and aging skin. But all peels are not created equal and their effectiveness and the depth to which they will go depends on the ingredients in your peel. Superficial peels typically use one or two active ingredients, such as alpha hydroxy acids (glycolic acid) or beta hydroxyl acids (salicylic acid) to remove the epidermis, or uppermost layer of skin. Medium-depth peels penetrate deeper into the epidermis.

Acne Attacks

Desquamation…sounds horrible, doesn’t it? But actually, desquamation is a very healthy process…the process by which your skin cells are shed and then replenished. The process is supposed to help keep your skin looking fresh and youthful. Unfortunately, when parts of the process go awry, it gives acne an opportunity to attack your skin on top of, and sometimes below, the skin’s surface.

It all begins many layers beneath the surface of your skin (the stratum corneum). There are five layers (or stratums) in all that your skin cells move through, the deepest of which is the stratum germinativum. This is where the cells begin and multiply, as they then make their way through three more layers (mucosum, spinosum, and granulosum) before reaching the surface. By the time your skin cells reach the “top”, they are, for all intents and purposes, dead cells and need to be removed to make way for the next batch that is on its way.

This whole process takes about 28 days; and in that time the dead skin cells that are awaiting exfoliation can become a part of mild to severe acne attacks or breakouts. It’s the combination of these dead skin cells, along with sebum and bacteria, that clogs the pores of your skin, creating blemishes.

These blemishes come in many shapes and sizes, from bumps to whiteheads and blackheads to papules and pustules, nodules and cysts. But the source goes back to your sebaceous gland, hair follicles, pores, dead skin cells and excess sebum.

The Importance of pH to Your Skin

In the past several years, a lot has been written and discussed regarding the pH in our bodies and the role acid and alkaline levels play in our health. Keeping pH levels in balance internally is vitally important to the functioning of your cells, but in order to give your internal system a fighting chance against disease and aging, your first line of defense begins with your skin.

In order for your skin cells to function properly, its pH levels need to remain at 4.5+/-1.0. Each day your skin wages a battle against forces that can, potentially, throw levels out of whack:

The environment and injuries it causes
Foods and liquids ingested
Waste products produced by the cells

The body’s natural defense mechanisms routinely work to maintain the pH levels through:

Neutralizing systems in the cells
Elimination of carbon dioxide by the lungs
Excretion of blood toxins by the kidneys

The skin’s ability to keep pH levels in line is aided by the mixture of sebum (oily fats) and sweat that coats the skin, creating an acidic barrier that fights bacteria and helps in the healing process should injuries occur.

As you age, however, the pH level of your skin becomes less acidic and more neutral, leaving it more susceptible to bacteria and infection. While this is happening, there are also biochemical changes taking place below the skin in the collagen and elastin that provide firmness and elasticity. As these changes progress, your skin becomes drier, less elastic, less supple and wrinkled. It also can be injured more easily and heals more slowly.

ANTI-AGING YOUR EYES

Laughing, smiling, fun days in the sun having drinks with friends…smoking, eating too much fast food, lack of sleep…typical activities for many of us; and the typical result of these activities on the thinnest skin on your body…the skin around your eyes...is dark circles, fine lines, and wrinkles. Of course, your gene pool does have some affect on how your skin ages, as well; but lifestyle choices can make a world of difference.

As we age, one of the first evident signs creeps in around our eyes. Those darn crows feet and other tiny lines start to give away the fact that we’re getting older when other parts of our skin still manage to remain somewhat youthful. It’s no wonder, then, that one of the cosmetic products that is highest on our ‘beautifying’ list is eye cream. But before we touch on the importance of eye creams in your skin care regimen, let’s not ignore other preventive maintenance that can be exercised that addresses the items mentioned in the beginning of this blog.

Sun – use a moisturizer with a minimum SPF 30; wear sunglasses with UVA/UVB protection properties.

Cigarette smoking – don’t do it! Nicotene and other properties in smoke dry out skin, while you squint as the smoke wafts by your eyes…not to mention the actual pursing of your lips that causes early wrinkles on that part of your face, as well.

Sleeplessness – the excessive production of cortisol in your body can wear down your skin cells. Not getting enough sleep is one cause of this cortisol production.

Alcohol – drying to your skin

Diet – What you eat is a huge factor in the appearance of your skin. Omega-3s and antioxidants are very good for your skin. It isn’t any secret that eating a healthy diet of fruits and veggies (antioxidants), coupled with fish (Omega 3) is much better for you (and your skin) than fast foods. Another favorite…cocoa…contains flavanols, a phytochemical that can help to improve circulation to your skin cells, improve its hydration and protect your skin from sun damage. Along with flavanols, carotenoids can also boost your skin’s ability to fight off wrinkles around the eyes. Foods that contain carotenoids are carrots, sweet potatoes, and spinach, just to name a few.

Genetics – well, although genetics can affect how your skin ages and the presence of dark circles, as they say…we can’t choose our relatives!

The dark circles, which plague many of us, can be a by-product of stored toxins, allergies, and lack of water in our systems. Increasing water intake and addressing allergies can possibly help reduce the intensity of these circles.

Now a word about the topical eye creams that can help with wrinkles, fine lines and dark circles. The older we get, the less our body manufactures collagen and elastin (in addition to hyaluronic acid), three important ingredients for that youthful glow we’re after. Without these proteins, our skin loses its elasticity and its firmness, causing the skin around our eyes to age quicker, evidenced by the increased wrinkles and sagging.

When searching for eye creams that will address the lack of collagen and elastin, ingredients that are proven to help are those containing peptides and antioxidants, such as vitamin C and A. KISSING LIP LINES GOODBYE

Although these cold winter months can do a number on lips, leaving them dry and chapped, they need to be given special attention throughout the year in order to minimize wrinkles and lines, keeping them soft and young looking.

Lips, as well as the skin around your eyes, are very sensitive and need a gentle, but effective, process to maintain their appearance. As we age, the skin in these areas begins to shrink and become thinner, which makes it all the more important to get into a good regimen of nurturing your lips early on. Practicing the following suggestions will go a long way towards having beautiful, healthy lips:

Eat plenty of fruits and vegetables
Drink ample amounts of water (6-8 eight oz. glasses/day)
Cut down on caffeine, as it has a dehydrating effect
Consider taking antioxidant supplements to aid in keeping lips soft
Give up smoking, if you currently smoke (it dries skin and reduces oxygen supply to skin)
Use lip balm with SPF
Apply topical moisturizers (with peptides, when possible)
Use lemon wedges to lighten brown spots

Just as you would with the other skin on your face and body, exfoliating your lips on a routine basis helps regenerate the skin by removing dead cells. Rather than use regular exfoliants, which can be too harsh for your lips, try this:

Apply your usual lip balm
Wet a washcloth, then dip it into a small amount of granular sugar
Gently scrub your lips
Wash off lips
Reapply lip balm

Types of Rosacea

As mentioned in my earlier blog post, there are millions who suffer from rosacea without even knowing it. The fact that there are four different sub-types of rosacea that can afflict sufferers helps to explain why there may be confusion regarding a “diagnosis” of certain skin problems and symptoms by those not familiar with them.

The first sub-type of rosacea is called erythematotelangiectatic rosacea…not nearly as hard to recognize as it is to pronounce! This sub-type is the most common and is what much of the population, in general, would think of when somebody mentions rosacea. It is routinely seen across the forehead, cheeks, and nose. Other symptoms of this sub-type are:

Telangiectasias (abnormal dilation of capillaries just below skin surface)
Flushing
Redness
Burning
Stinging
Swelling

Sub-type 2 of rosacea is called papulopustular rosacea, which pretty well describes the symptoms of this sub-type…papules (bumps), pustules (pimples), and red patches. It has been called the “classic” sub-type. Papulopustular rosacea very much resembles acne (without the blackheads) and is sometimes called acne rosacea. Again, those dealing with Sub-type 2 rosacea may experience stinging or burning.

When talking about phymatous rosacea, sub-type 3, the most recognizable symptom is the development of excess tissue in areas of the face, including the chin, ears, forehead, cheeks, and…most significantly…the nose (think W.C. Fields). In addition, thickening of the skin in these areas, as well as nodules, may also occur.

Ocular rosacea, the final sub-type, can be evidenced by the following:

Bloodshot and watery eyes
Dryness and itching
Burning
Blurred vision
Swollen eyes
Light sensitivity
Feeling of having object in the eye
telangiectasia (see definition above) of the conjunctiva (mucous membrane lining of the inner eyelid and part of eyeball) and lid margin.

Obviously, with any of the symptoms exhibited by these four sub-types, a physician should be consulted. There are numerous treatment options for rosacea (see last week’s blog post), and leaving it untreated could lead to worsening of the condition.

TREATING... AND LIVING WITH... ROSACEA

For millions of sufferers, the fact that there is no known cure for rosacea can be disheartening. The good news is the condition is definitely treatable. In fact, by not treating rosacea you run the risk of having the symptoms worsen.

It is definitely important to work with a physician who can help with choosing the appropriate treatment plan, depending on the type and severity of your rosacea. The treatment options for rosacea can include antibiotics, topical medications, anti-hypertensives, lifestyle changes, and even laser treatments.

Rosacea is an inflammatory condition; and antibiotics can help to reduce inflammation, as well as inhibit blood vessel formation, which is at the heart of this skin problem. As is the case with most antibiotics that are prescribed, they aren’t intended for open-ended use, most especially because of the fact that your body can become resistant to the medication…not to mention that there are usually numerous side effects that can accompany antibiotics. Once the more irritating and severe symptoms have subsided, other treatment options can help keep the rosacea under control.

Working with your physician, a treatment plan can be created that might include other oral medications (aside from antibiotics) and topical medications. These options can help to reduce the redness and inflammation and, depending on what is prescribed, might also work on oil-producing glands in the skin to lessen outbreaks of acne rosacea. When using topical medications, a good rule of thumb is to wait after washing your face approximately 30 minutes prior to applying them to avoid stinging. Give the medication another few minutes to dry if you are then going to apply moisturizers, make-up, etc.

One option that can be worth researching with your physician is laser treatments. These treatments can help reduce the blood vessels that start to become more visible under the skin and possibly other symptoms, such as “pimples”; however, it is extremely important to enlist the counsel and services of a physician who has experience and is skilled in the use of lasers for rosacea therapy.

Lifestyle changes can be as simple as changing how you wash your face to something more challenging, such as controlling the stress in your life.

When cleansing your face, use lukewarm water rather than hot, use your fingertips rather than abrasive washcloths or scrubbing sponges, avoid products that contain alcohol and fragrances, and if you are shaving your face, consider using an electric shaver in lieu of a typical razorblade shaver.

Other lifestyle changes involve trying to avoid, as much as possible, the triggers that were discussed in last week’s blog: sun exposure, hot drinks, red wine, spicy foods, etc.

In 2007, the National Rosacea Society conducted a survey to determine how rosacea affects sufferers emotionally and mentally in their daily lives. The survey results showed that:

76% had lower self-confidence and self-esteem
70% of those with severe symptoms were negatively impacted professionally when interacting with others
38% shared that they avoided social engagements and being out in public
30% revealed it caused them to miss work

The vicious circle this can create is that stress can aggravate rosacea and the aforementioned results of having rosacea can cause that stress. Learning stress reduction techniques can certainly be of benefit. Some patients have been prescribed anti-hypertensives to lower their blood pressure (high blood pressure is a trigger), which can reduce certain symptoms (such as severe flushing); but this most definitely is not an option for everyone and must be seriously discussed with your physician.

Treating rosacea and minimizing the symptoms are very doable. If you suffer from rosacea, you owe it to yourself to learn as much as you can about the condition, and talk with your physician to find out what might be the best treatment for you.

THE FACTS ABOUT ROSACEA

Rosacea…most of us have heard of it at one time or another, however, most of those who suffer from rosacea aren’t even aware that they have it. A good number of the population most relates rosacea with redness that occurs on the cheeks. The fact is Rosacea affects those who suffer with it in many different ways.

Rosacea is a chronic, inflammatory skin condition that affects as many as 16 million Americans. If you are over 30, have fair skin, are a woman and have some family history of rosacea, you are at higher risk of developing rosacea. When rosacea occurs in men, it can often manifest in a more severe way, possibly because it hasn’t been treated in as timely a fashion.

The signs and symptoms of rosacea can include:

Blush or flush easily (this is one of the earliest signs)
Bumps/pimples (similar to acne)
Burning or stinging
Edema (swelling)
Irritated eyes and swollen eyelids
Dry and itchy facial skin
Plaques - redness and thickening
Small and visible blood vessels

Although the actual cause of Rosacea is still something being researched, there are well-known triggers, starting with the following foods:

Cheese
Eggplant
Beans (such as lima, navy) and peas
Citrus (as well as tomatoes, bananas, plums, raisins)
Spinach
Spicy foods
Vinegar
Vanilla
Yogurt

On the plus side, eating whole grains, blueberries and cherries can actually help reduce some of the redness of rosacea.

Along with avoiding the foods listed above, individuals with rosacea should minimize exposure to the sun (use at least SPF 30 sunscreen), wind, and high temperatures. Red wine and hot drinks are also known triggers and removing them as a routine part of your diet should be considered.

Add menopause and high blood pressure as two other conditions that may cause flare-ups, and you have a pretty large laundry list of things to look out for if you are dealing with rosacea.

Although there isn’t a cure for rosacea, there are things that can be done to treat it. We’ll discuss these in our next blog post.

Athlete's Foot - Proper Treatment

Question:

My son is in high school and is playing football. With the two-a-day practices and showering in locker rooms, he has developed a red itchy rash between the last three toes on his left foot and a similar rash between the fourth and fifth toes on his right foot. He says that it is very itchy. His coach told him it was athlete’s foot, and he should get some cream at the drug store to treat it. He said it is common for athletes to get athlete’s foot, and there was no reason to see a doctor. Should the football coach stick to coaching football rather than giving medical advice?

Answer:

Athlete’s foot is a popular term used to describe a fungus infection of the feet. Although one can try over-the-counter, anti-fungal medications for scaly, itchy rashes between the toes, not all rashes between toes are fungal.

If one wants to be precise and scientific about a foot rash, a physician, especially a dermatologist, can do a microscopic exam of the peeling skin between the toes to identify the cause. Fungi grow the best in warm, moist areas, which makes between the toes and in the groin areas prime locations in athletes. Feet that are enclosed in socks and shoes suffer from increased sweating during physical activity and can encourage the growth of fungus on other areas of the foot.

As a rule, it is felt that infections are acquired from other people, however, with fungal infections this is not always the case. Experimental efforts to deliberately infect healthy skin have failed, and a husband and wife may live together for years with only one partner having a fungus infection of the foot or toenail. The importance of susceptibility of the skin and exposure to infected surfaces plays an important part in one’s ability to acquire a fungal foot infection.

It is important to distinguish between fungal infections of the foot and conditions such as psoriasis, eczema, and allergic reactions to shoes or laundry products for athletic clothes. Initial treatment of properly diagnosed fungal infections of the feet, i.e., athlete’s foot, requires treatment of the fungus and elimination of the warm, moist environment that promotes fungal foot infections. Showers cause the skin on the feet to become moist. A towel does not adequately dry the skin of the web spaces between the toes. About 10 minutes of air drying or the use of a blow dryer on a cool temperature setting is required.

Many anti-fungal medications are available over-the-counter for clear cut, uncomplicated fungal toe infections. With severe or unresponsive fungal foot infections, oral medications may be indicated and require a doctor’s prescription. Remember, an accurate diagnosis is critical for effective treatment and results.

Breast Cancer Scars and Vitamin C

Question:

I am a 49-year-old woman with breast cancer currently undergoing chemotherapy after my surgical treatment. During my research on line, I read information related to the topical use of vitamin C to improve skin healing related to breast cancer surgery, chemotherapy, and radiation treatments. Can you explain the science related to the material I have read on the internet?

Answer:

Ultraviolet rays and other inflammatory insults, such as cigarette smoking and pollutants, create free radicals (molecules that poison DNA, proteins and lipids) that cause photoaging of the skin. It has been demonstrated that the skin’s native antioxidant protection (molecules that neutralize the free radical poison) breaks down during excessive UV injury and promotes aging due to the fact that there are more free radicals created by the UV damage than there are antioxidant molecules available in the skin. This allows free radicals to damage DNA, proteins, and lipids causing skin cells to die.

With this background information, it should be no surprise that breast cancer, chemotherapy, radiation, and medication required post-breast cancer treatment create a large free radical burden on the body and skin.

Vitamin C is an essential vitamin. The body cannot synthesize L-ascorbic acid, so humans rely on dietary intake or topical application for their supply of vitamin C. It must be ingested from vitamin supplements, food sources (such as citrus fruits, dark-green leafy vegetables, strawberries, tomatoes, green and red peppers, brussel sprouts, and turnips) or applied topically to the skin to replenish L-ascorbic acid in the body and skin. Vitamin C is essential for normal cell growth, collagen synthesis, and wound healing.

Oral vitamin C (L-ascorbic acid) cannot deliver an adequate amount of L-ascorbic acid to the skin (epidermis). Conversely, topical vitamin C (L-ascorbic acid) cannot deliver an adequate amount of vitamin C to the body.

The skin relies on antioxidants for protection against free radicals. Since the skin receives the most free radical assault from ultraviolet light exposure, cancer, and the effects related to cancer treatment, replenishing and increasing the antioxidant defense of the skin becomes an attractive strategy for wound healing. Excessive Underarm Sweating

Question:

It has been a very hot summer in St. Louis which has made me realize just how severe and embarrassing my armpit sweating has been. I had to change blouses several times a day as my sweating stained the armpits of the blouses and in some cases ruined blouses creating a huge expense just buying new clothes. I tried every deodorant and antiperspirant on the market including those for men without any benefit. I see ads on TV that claim there is a treatment for excessive sweating. Is that true?

Answer:

There are people who have abnormal excessive armpit sweating. The medical diagnosis for this condition is called “axillary hyperhidrosis.” Most patients suffer with this condition for years before seeking medical evaluation. The embarrassment and feeling that nothing can be done to treat the condition causes patients to delay evaluation and treatment options for the excessive sweating. Severe axillary hyperhidrosis is a debilitating disease characterized by markedly excessive sweating which severely limits patients’ abilities to carry on daily activities. There are tests to measure the severity of axillary sweating, however, this is usually confirmed by history and physical examination of clothing.

The psychological impairment includes unwillingness to participate in physical activities and social events. Economic loss includes the destruction of blouses, shirts, sweaters, and other clothing and excessive cleaning bills. I think the TV advertisements have created an awareness that there are a number of treatment options for excessive armpit sweating if over-the-counter preparations are not effective. This requires evaluation by a physician trained in the treatment of this condition.

Dermatologists are the most qualified in this area, however, some other physicians may have expertise treating excessive armpit sweating. There are topical prescription medications, such as drysol solution and other topical preparations. There are also oral medications used to treat excessive armpit sweating, however, they may have unwanted side effects.

With a diagnosis of severe axillary hyperhidrosis and failure to respond to oral and topical treatment, Botox injections are an effective alternative. Botox, botulinium type A, was approved by the US Food and Drug Administration in 2004 for the treatment of severe primary axillary hyperhidrosis that is inadequately managed with topical and oral medication. This treatment is very effective, but needs to be repeated at about six-month intervals. Reducing the Appearance of Scars

Question:

I am a 38-year-old woman; and I was working in the yard and cut my thigh on a limb. It bled for several minutes; but the cut did not seem very deep, and I did not get stitches. I kept the area clean, and it did not get infected. I now have a red scar that feels bumpy. How can I make the scar disappear?

Answer:

The body’s natural healing process of wounds caused by an accident or surgery is a scar. The more the skin is damaged, and the farther the wound is away from the heart, the longer it takes to heal and the greater the chance of a noticeable scar. Generally, the scar is most prominent within the first few weeks of the injury and gradually improves over several months. Based on a combination of these types of factors, many wounds that heal, although quite healthy, appear cosmetically undesirable.

Obviously, the initial treatment of a cut has a major impact on its healing and residual scar. Several techniques can alter or camouflage the appearance of the resultant scar. However, no scar can ever be completely eradicated. Conservative measures include sun protection, scar massage and good skin care of the injured area. Ultraviolet ray damage from the sun or tanning beds can darken scars and should be avoided. Sunscreen and covering the area can accomplish this goal. Scar massage with various topical products has been shown to improve scars. A list of these products includes:

· Vitamin E preparations

· Mederma

· Vitamin C topical products

· Protein moisturizers

The gentle scar massage process with any topical moisturizer tends to improve thickened scars. It is important to avoid trauma or skin stretching to new scars, as this tends to increase the size and thickness of the scar.

After several months, if scars remain a cosmetic problem, dermatologists and plastic surgeons have several techniques that can be performed to alter the appearance of scars. Modern techniques can change the length, width or thickness of a scar; but no technique can be expected to return the skin to its original appearance before the scar.

Scabies - A Souvenir from Florida?

Question:

I am 38 years old, married, and have four children. My sister who lives in Miami called and informed me her son has scabies, and she is treating her entire family. In addition, she was given instructions on cleaning clothes and her house. I have not seen my sister or her family since the Christmas holidays. Should I be concerned for my family?

Answer:

Scabies, or the “itch mite”, is a tiny, microscopic skin mite which burrows into the skin. The mite does not fly, which means it requires very close personal contact with a person who has scabies to be contagious.

The mite prefers intertriginous areas (web spaces between the fingers and toes, under the breast and breast area, armpits, waist and navel area, and the genital area) as its initial site of attack. After a few weeks of infestation (3-4 weeks), a person begins to develop an allergic reaction to the mite, its eggs, and its excretions. The rash usually begins near the site of the initial infestation, which includes the hands, wrists, breasts, armpits or genital area. In severe cases, the allergic reaction rash can spread to most of the body.

This does not mean that there are mites in all areas of the rash. They tend to stay in the skin crease areas mentioned above. The physician will try to identify mite burrows in skin crease areas. A skin scraping for a microscopic exam from these burrows will allow the doctor to identify the mite, which confirms the diagnosis. Most itchy rashes are not scabies; and it is very important to make an accurate diagnosis, as all close personal contacts, family members in the same household, and sexual partners should be treated at the same time.

Treatment consists of applying a mite killing medication to the skin, cleaning all clothing worn within the last 24-48 hours, and striping all bed linens for cleaning as directed by your doctor. The mite requires human skin for survival. That means the mite cannot live for more than 24-48 hours without a human host. Any clothes and furniture not used for 24-48 hours do not require washing or fumigation.

The topical medications for scabies should be applied no longer than the time directed by your doctor, as the medication can irritate the skin, causing a rash and itching. The rash and itching may continue even though the mite has been killed, since the rash and itch represent an allergic skin reaction to the mite and its excretions. The residual rash and itch can be treated with anti-itch cortisone creams and oral anti-itch pills.

The itchy rash of scabies usually clears up in 2-6 weeks with an accurate diagnosis and treatment program. Obviously, treatment of an itchy, scaly, red rash that is not scabies with anti-scabetic medication will only aggravate the rash and cause other family members and close contacts to get unnecessary treatment that may cause them to get a rash from the topical anti-mite medication. An accurate diagnosis prior to treatment is mandatory.

Your contact with your sister’s family has been several months. If no family members are itching and there is no rash, it is most likely that you have no reason for concern and should not treat yourself and your family. You might ask your sister if the doctor actually scraped the skin of her child and identified the mite, as doctors see a lot of cases which get treated without an accurate diagnosis.
